Office Policies

To schedule an appointment please call (407) 673-4600, we will make every effort to accommodate your busy schedule.

Initial visit
Please make sure you bring the following items with you when you arrive for your appointment:
Insurance card(s).
A form of personal identification.
Primary care physician referral form.
Your previous medical record including previous blood test results.
A list of all your medications including dose and frequency.
Co-Payment due at the time of your visit.

Please arrive at least 15 minutes prior to your appointment in order to complete the necessary paper work.

Follow up visit
Please notify our staff if there are any changes in your medical health insurance, name, address, telephone number and any other pertinent personal or medical information.

Cancellation / late arrivals
If you are unable to keep your appointment, we ask that you kindly provide us with at least 24 hours advance notice. This courtesy, on your part, will make it possible to give your appointment to another patient. If sufficient notice is not provided you may be charged for the scheduled visit.

Prescription refills are made only during the office visit. We will not refill prescriptions via telephone. Please have your pharmacy fax a request. This is the best way to insure that quality control measure are being met and your health is closely monitored by our staff. Please keep this in mind when planning for your medication needs especially prior to weekends and holidays. There is a 24 to 48 hour turn around time for prescription refill requests.

Test results
We will notify you of your test results within 2 to 3 weeks. If we do not contact you during this time frame, please call our office to inquire.

Medical Records
To obtain copies of your medical records you may contact our medical records department. We will require you to sign a medical records release form. There will be no charge for sending your records directly to another physician. However, there will be a nominal charge for copies of medical records in all other circumstances for which payment is required in advance. We need ten working days to complete all medical records request.

Financial Policy
We provide insurance billing as a courtesy for our patients. Co-payments are due at the time services are rendered. Please be aware that any costs not covered by your insurance are your responsibility and we request payments at the time of your appointment. Forms of payment accepted by our office are check, cash, Visa, & Mastercard.

We accept most insurance plans including Medicare. For more details call our office and ask one of our staff members.
